Athletic Medical Clearance Process for New Varsity Athletes

All freshman, transfers, and new members to Central varsity athletics must be medically cleared for sport participation. As your health and safety are of the utmost importance you must receive medical clearance from our office, Central's Student Health Services at the Student Wellness Center, prior to participating in our varsity athletic program. Our athletic medical clearance process requires you to complete the following:

  1. Complete the 'Connecticut State University (CSU) Student Health Services Form'
    Found on-line in MEDICAT (Click the "Forms" tab)
  2. Completion of Immunizations Record as noted on the MEDICAT Home Page to include:
    1. UPLOAD your Immunization Record from your PCP to “IMMUNIZATION SECTION”
    2. Enter an up-to-date TETANUS date to “IMMUNIZATION SECTION” and UPLOAD of the supporting document.
    3. Submission of Sickle Cell Trait (SCT) Lab - Requires an UPLOAD of SCT LAB RESULT to “IMMUNIZATION SECTION”.
  3. Complete the Central Varsity Athletics: Supplemental Student Health Services' Form (Blue 3-Pages).
  4. Once all your forms are reviewed and considered complete, an appointment with Central Student Health Services for Sport Clearance will be scheduled.

INSTRUCTIONS FOR COMPLETING THE Central VARSITY ATHLETICS: SUPPLEMENTAL STUDENT HEALTH SERVICES FORM 

(BLUE FORM 3-Page PDF)

Step 1: Your Sport Pre-Participation Physical Exam must be conducted by your primary care provider (PCP). 

_____ Please schedule an appointment with your PCP office as soon as possible. Please note that as per NCAA requirements this physical exam must be done WITHIN the last 6 months of the final clearance for your sport by Central Student Health Services.

Step 2: Health Questionnaire Section (Pages 1 & 2 of the 3-page Blue Form) - To be completed by YOU.

_____ Please complete the health questionnaire section - pages 1 & 2 section prior to your sport pre-participation physical examination (PPE) with your PCP. You may need assistance from your parent(s)/guardian(s) to complete this section, as an accurately completed history form is essential to this process.

Step 3: Pre-Participation Physical Examination (PPE) - (Page 3 of the 3-page Blue Form) - To be completed by your PCP.

____ You must bring the entire 3-page Central VARSITY ATHLETICS: SUPPLEMENTAL STUDENT HEALTH SERVICES FORM (BLUE) to your PCP to complete. Please note that Central Health Services will not accept any other forms or copies of records in lieu of this 3-page form. Be sure that your PCP signs BOTH page #2 and #3.

Step 4: UPLOAD THIS COMPLETED 3-PAGE Central VARSITY ATHLETICS SUPPLEMENTAL FORM - UPLOAD Section of MEDICAT under the heading CSU Health Information Form.

Note: Your PCP may recommend further testing/labs for any conditions found at the time of your PPE exam. Please make arrangements to have the recommended testing/labs done at home before your anticipated date of arrival. Since many times insurances will not cover out of state providers/and or services, it is important to have all testing done prior to your arrival at Central.

If in the past, you have had any diagnostic tests i.e., cardiac, respiratory, or any other medical workups, then results must be submitted with your forms. Obtaining Lab Results for Sickle Cell Trait (SCT)

Please contact your primary care provider to obtain a copy of your newborn screen test results or request that your PCP order labs for a new sickle cell screening test.

The NCAA requires that prior to participation in any intercollegiate athletic event (including strength and conditioning sessions, practices, competitions, or try-outs), each new, first-time student athlete must either show proof of a prior test for sickle cell trait or be tested for sickle cell trait. Most states started screening all newborns by 1990. As of April 2022, the NCAA no longer permits a waiving the test results for SCT. 

Tetanus - Valid

Athletes are required to maintain a valid tetanus vaccination within the current academic year. Tetanus is valid for 10 years.

Please retain a copy of all forms submitted into MEDICAT for your own records.

IMPORTANT:

You can avoid delays in being medically cleared to participate in your sport by completing all necessary medical assessments at home and uploading into MEDICAT at least 2 months prior to your sports anticipated first day of practice and/or preseason conditioning sessions.

DO NOT email, fax, mail or give medical health forms to coaches to submit for you. Your coaches should not request or be provided with copies of your personal medical health forms. It is your responsibility to submit the medical forms directly to Central Student Health Services via MEDICAT.
