Health Forms and Required Documentation

All students including graduate, undergraduate, transfer, residential and commuter students must complete the CSU Health Services Form by July 15th (summer and fall admits) and December 15th (spring admits). These are all submitted via the Medicat Portal. Please click here to access Medicat (link to medicat page).

  • ALL students are required to have two of the following vaccines: MMR (measles, mumps and rubella) and Varicella (chicken pox). 
  • ALL RESIDENTIAL students are also required by the State of Connecticut to have the meningitis conjugate vaccine 5 years prior to their move-in date.
  • ALL students who are completing the health forms must fill out section 6 regarding tuberculosis risk assessment to determine if further documentation is required.

Immunization/Screening Requirement Details

You must provide proof of one of the following:

-TWO measles (or MMR) immunizations: one after your first birthday and one at least 28 days later, OR
-Documentation of a positive measles titer (blood test), OR
-Documentation of date of measles disease by your healthcare provider on the CSU Health History form

You must provide proof of one of the following:

  • TWO rubella (or MMR) immunizations: one after your first birthday and one at least 28 days later, OR
  • Documentation of a positive rubella titer (blood test), OR
  • Documentation of date of rubella disease by your healthcare provider on the CSU Health History form

You must provide proof of one of the following:

  • TWO mumps (or MMR) immunizations: one after your first birthday and one at least 28 days later, OR
  • Documentation of a positive mumps titer (blood test), OR
  • Documentation of date of mumps disease by your healthcare provider on the CSU Health History form

If you were born on or after 01/01/1980, you must provide proof of one of the following:

  • TWO varicella immunizations (one after your first birthday and one at least 28 days later), OR
  • Documentation of positive varicella titer (blood test), OR
  • Documentation of date of varicella disease by your health care provider on the Health History Form.

Complete the “Tuberculosis (TB) risk assessment” on your student health portal. If you answer “yes” to any questions you will need a TB test. All Tuberculosis testing must be done within 6 months of your matriculation date.

If required there are two acceptable testing methods:

  1.  Interferon-Gamma Release Assay (IGRA) This is the preferred method. The two IGRAs that are commercially available in the U.S. are:
    1.  QuantiFERON®-TB Gold  (QFT-GIT)

    2. T-SPOT® (T-Spot) 

      OR

  2. Mantoux tuberculin skin test (PPD)

If your TB test is positive, or if you have had a positive TB test in the past, you will need a chest x-ray. We accept chest x-rays done within the past 6 months. The radiology report MUST be attached in order for us to accept the x-ray result.

*The Meningococcal requirement applies only to Residential students*

Meningococcal (MCV4) – Students living in university-owned housing must provide proof of receiving at least one dose of meningococcal conjugate vaccine not more than 5 years before enrollment.
